Chapman rose to head home a Chris Kamara cross in the second half as Leeds won the title from Sheffield United on goal difference.
**On this day in 2006, a late goal from Eddie Lewis rescued a draw for Leeds in the first leg of their play-off semi-final against Preston North End at Elland Road.
